Infertility Assesment

Infertility assessment involves a series of tests and evaluations to determine the possible causes of difficulty in conceiving. It typically includes medical history, physical examinations, hormone testing, imaging studies, and specialized fertility tests for both partners. Here’s an overview:

For Women:

  1. Medical History & Physical Exam:
    • Menstrual cycle regularity
    • Previous pregnancies or miscarriages
    • Pelvic pain, infections, or surgeries
    • Lifestyle factors (smoking, weight, stress)
  2. Ovulation Testing:
    • Basal body temperature (BBT) tracking
    • Ovulation predictor kits (LH surge detection)
    • Blood tests for hormone levels (FSH, LH, estrogen, progesterone, prolactin, AMH)
  3. Imaging & Structural Evaluation:
    • Ultrasound (to check ovaries, uterus, follicles)
    • Hysterosalpingography (HSG): X-ray to assess fallopian tubes and uterus
    • Sonohysterography: Uses ultrasound and saline to examine the uterine cavity
    • Hysteroscopy/Laparoscopy: For detecting endometriosis, fibroids, adhesions
  4. Ovarian Reserve Testing:
    • Anti-Müllerian Hormone (AMH)
    • Antral follicle count via ultrasound
  5. Genetic Testing:
    • If there’s a history of recurrent miscarriages or suspected chromosomal conditions
    •

For Men:

  1. Medical History & Physical Exam:
    • Testicular pain, injury, surgeries
    • Sexual function (erectile dysfunction, ejaculation issues)
    • Lifestyle factors (alcohol, smoking, obesity, medications)
  2. Semen Analysis:
    • Sperm count, motility, morphology, volume
    • Semen pH and white blood cell count (for infections)
  3. Hormone Testing:
    • Testosterone, FSH, LH, prolactin levels
  4. Genetic Testing:
    • Karyotyping or Y-chromosome microdeletion testing if low sperm count is detected
  5. Imaging Tests:
    • Scrotal ultrasound (to check for varicocele, blockages)
    • MRI (if needed for deeper anatomical issues)

Additional Testing (If Needed for Both Partners):

  • Postcoital Test (PCT): Evaluates sperm survival in cervical mucus
  • Endometrial Biopsy: Checks uterine lining for implantation readiness
  • Antisperm Antibody Test: Detects immune reactions against sperm
  • DNA Fragmentation Test: Assesses sperm DNA integrity

Next Steps:

After assessment, treatment options may include lifestyle changes, medications (such as Clomid or gonadotropins), surgical correction, assisted reproductive technologies (IUI, IVF), or donor options depending on the underlying cause.
